Payroll forms may be printed from this web site. Please click on the desired form and print a copy. Fill out the appropriate information. Mail or hand deliver the form to Frostburg State University's Payroll Office. Please remember that the contractual and regular payrolls are two separate systems. If you are on both payrolls, forms have to be done separately for each payroll respectively.

  • Direct Deposit form (viewing note)

    Remember the following:
    • Direct deposit will take at least two pay periods to take effect.
    • Cancellation of an old account will occur within 21 days of receipt at Central Payroll Bureau; you will receive one or two payroll checks until the new account is established. Do not close the old bank account until a payroll check is issued.
    • Discontinuation of direct deposit will occur within 21 days. Do not close your bank account until a payroll check is issued.

  • I-9 form (Employment Eligibility Verification) (viewing note)
    The I-9 form is used to verify that an employee is eligible to work in the United States. Completion of the form should be done prior to the employee's start date. Section 1 of the I-9 form is to be completed by the employee. For a student employee, section 2 should be completed by the supervisor or by an employee in the Payroll Office. Student employee's I-9 forms are sent to the Payroll Office. Contingent I and Contingent II employee's forms are housed in the Office of Human Resources. Acceptable documents are listed on the form for your convenience.

  • Payroll Change of Address form & Savings Bonds form & Savings Bond Instructions
    An employee's address needs to be kept current with the Payroll Office. W-2 information at the end of the year is sent to the address on the check. Make sure the address on your check is correct.

    Additional savings bond information can be found at www.savingsbonds.gov.
  • W-4 form (viewing note) & W-4 Worksheet
    An employee may change their W-4 anytime during the year. You may want to complete the worksheet before filling out your W-4.
