Allergy Injection Information

Three forms regarding allergy injections given at the Brady Health Center must be read/completed as needed and on file prior to your receiving allergy injections.

1. Allergy Injection Information Sheet (.doc)

Student must read, complete and return the form to the Brady Health Center.

Please forward the following to your allergist:

3. Physician Information Sheet (.doc)

The physician (allergist) must read the attached letter, complete the information sheet and return it to the Brady Health Center in order that you may receive your allergy injections next semester.

It is imperative that you bring paper work and allergy medication to the health center at least 2 days prior to the day receiving your first allergy injection at the center. If you or your allergist have any questions or concerns, please do not hesitate to contact our office at 301-687-4310.
