Immunizations

Copy of your immunization record is required to be on file at Brady Health.

Required immunizations: MMR (2), Tetanus Diphtheria (or Tdap) booster within the last ten years,and completion of the Tuberculosis Risk Assessment Form. Those who have any positive responses on the risk assessment form must have a PPD (tuberculin) skin test (within the past year). Students with a positive PPD skin test (current or a history of) must have a chest x-ray and forward a copy of the official chest x-ray report.

Health holds are placed on student records without required immunization documentation, prohibiting registration until complete.

Strongly Recommended immunizations: Hepatitis B vaccination series, Varicella, Gardasil and the Meningococcal vaccinations.

Meningococcal Vaccines (Menomune or Menactra) , Hepatitis B Vaccine, Td/Tdap vaccine(s) (tetanus/diphtheria/acellular pertussis), Gardasil and PPD skin testing are available at Brady Health Center by appointment.

Flu Vaccine is available at the Brady Health Center (in season)and in local clinics.
