Requesting Disability Services

A student is responsible for formally requesting disability services in writing through one of the following means.

• The student can complete and submit an SSS/DSS Information Form. This form is sent to all newly admitted students who have indicated their intent to enroll at FSU.
  
    or
   
• The student can submit documentation of disability.
   
   or
   
• The student can submit a letter requesting disability services to:

In order to be formally accepted into the DSS program, documentation of disability must have been submitted. It is a good idea to submit documentation when requesting services.

DSS will only accept requests for disability services from currently enrolled FSU students or students who have formally applied to the University through the Office of Admissions.
