Tutoring Forms

This page allows students and potential tutors to access the form(s) they need.  Simply download, complete, and take the form(s) to the Tutoring Center located in Pullen 151.  The forms are in .pdf format.  If you have difficulty downloading the appropriate form, you may pick it up at the Tutoring Center.  Below is some important information about the Tutoring Request forms:

  • Write legibly
  • Fill out the entire form
  • Be accurate when  indicating the course name and number
  • Provide FSU e-mail account

The Tutor Pairing Process

The FSU Tutoring Center offers three types of tutoring:  individual writing, individual content, and group.  How students get paired with tutors depends on tutor availability and what type of tutoring is requested. 

  • If individual writing tutoring is requested, students need to make an appointment at the Center.  This can be done in person or by dialing x3077.  At times, walk-ins can be accommodated.
  • If individual content tutoring is requested (math, biology, etc.), students will be paired with a tutor via e-mail.  
  • If group tutoring is requested, students simply attend the session on the designated day and time.

The Center staff does its best to fill requests in a timely manner.  However, there are times—especially mid-term —when the pairing process takes a while.  We ask that students be patient during this busy time.
