INSTRUCTIONS FOR USE OF TSR FORM

This form is required to be used whenever the following telephone related items are requested:

  • Addition of a new telephone line
  • Moving an existing line from one location to another
  • Changing an existing line from an analog line to a digital line or vice versa
  • Modifying the 'dial out' features of an existing line
  • Deletion of an existing line

This form will allow you to make an online Telephone Service Request (TSR) and then submit it to the Department of Networking & Telecommunications. You may submit three requests per TSR form. Once you complete a submission, you will have the option to return to this page and complete additional requests. Any service request not available on this form should be submitted through the FSU Help Desk.

Using the form is very simple:

  • Scroll down this page to the form
  • Fill in all required information fields
  • Use the 'Service(s) Requested' section to detail what work you need completed
  • Click 'Submit' to send your request
