Library Instruction Request Form

Instructors may use the form to schedule subject-specific bibliographic instruction, ORIE and ENGL 101 class sessions with a Faculty Librarian in the Library's Instruction Center. Please note that there is an expectation that the instructor attend the session(s) along with the students.
