Benefits> Procedure for Reporting an On-the-Job Injury

Action to be taken when an employee injury is reported:

Notify Lee Ann Nightingale at the Office of Human Resources immediately at extension 4105 when an employee is injured or a near miss occurs.

When an employee is injured or involved in a near miss accident, the employee and the supervisor, independent from each other must complete the "Employee's First Report of Injury" (Adobe Acrobat - .pdf - Format) forms as soon as possible.

When completed send the original forms to the Office of Human Resources. If you have any questions, call the Office of Human Resources at extension 4105.
