Risk & Safety Management > Procedure for Reporting an On-the-Job Injury

Action to be taken when an employee injury is reported:

Notify Rosemary Hall at the Office of Human Resources immediately at extension 4105 when an employee is injured or a near miss occurs.

When an employee is injured or involved in a near miss accident, the employee and the supervisor, independent from each other must complete the "Employee's First Report of Injury" (Adobe Acrobat - .pdf - Format) forms as soon as possible.

When completed send the original forms to the Office of Human Resources and a copy to Jim King. If you have any questions, call Jim King at extension 4897.
