Student Athletes

Welcome student Athletes. We encourage all athletes to have their sports physical completed by their primary care provider.

1. Sportsware

If you plan to have your Sports Physical done at the Brady Health Center, you must bring your completed Sportsware medical history forms with you to your appointment at Brady Health Center. Prior to scheduling an appointment for an athletic physical, ensure you have received an acceptance email from athletics and are able to access Sportsware and print off the completed medical history forms. If you fail to bring these completed forms with you, your physical exam may be rescheduled.

  • Prior to scheduling an appointment for an athletic physical, ensure you have received an acceptance email from athletics and are able to access Sportsware.

  • Complete all your forms on Sportsware.  Be sure to “save” each form when completed.

  • After electronically signing it, print and bring the forms to your scheduled appointment. 

  • Sportsware Instructions: 
    • Go to the Sportsware site - www.swol123.net
    • Sign in
    • Click on “Forms” on the top black banner
    • Click on “Medical history”- this is the first form listed
    • Select “Open” on the left hand side
    • Print-total of 4 pages

  • If any problems with Sportsware access or log-in, please contact your coach or athletic trainer right away.

2. Sickle-Cell Trait Blood Test

All student athletes must have proof of a sickle-cell trait test. If you are unable to locate your results, you can have your primary care provider perform the test or you can schedule this test at Brady Health Center.

All students born in the United States after 1990 had this test done as part of their newborn blood screening.  Some of these tests may be referred to as “Newborn profile”, “Hemoglobin electrophoresis”, “Hemoglobin S solubility”  as well as “Sickle-Cell screening”.   To avoid repeating this test, you should attempt to locate the results.

Some suggestions for locating the results:

  • Medical Records at the hospital in which you were born
  • Pediatrician
  • Primary Care Provider
  • Parents or guardians

If you are UNABLE to locate your results, you will need to have this test repeated. It is a blood test, which can be done at your primary care provider’s office, or at Brady Health Center by appointment.

If you choose to have this completed at Brady Health Center:

  • Bring your insurance card to the appointment or a copy of the card (front and back). 

  • Results can take up to 1 week, therefore please ensure you have this completed prior to the start date of your sport. Failure to complete the sickle-cell testing may delay your participation in the sport.
