Life Insurance Benefits
Frostburg State University employees have two plans available.
- USM benefits plan
- Term Life
- Long-term Disabilty Insurance (income protection plan)
- State of Maryland benefits plan
- Term Life
- Accidental Death and Dismemberment Plan
Metlife Plan Comparison for Life Insurance between USM and State of Maryland
All regular employees and contingent II employees have the option of enrolling in either or both plans. The plans are independent of each other and require separate enrollment documents.

MetLife is the provider of your Group Term Life Insurance and Long-Term Disability (LTD) Program.
Term-Life Insurance
Coverage for Yourself
If you are a newly eligible employee you may elect coverage in $10,000 increments up to the lesser of
6x salary or $750,000.Coverage for Your Spouse
You may chose to purchase coverage for your spouse who is eligible in $10,000 increments to the lesser of 100% of the employee’s amount or $150,000,Coverage for Your Dependent Child(ren)
You may chose to purchase coverage for your dependent child(ren) eligible at the following amounts:
Birth to 6 months: $1,000

Long-Term Disability
Long-term disability (LTD) benefits are available through MetLife. The LTD plan provides employees with a percentage of their gross monthly salary if they cannot work because of a covered injury or illness. Employees can enroll at any time; medical underwriting is required if an employee enrolls after the first 60 days of employment. (The provider will mail a medical statement upon receipt of the application.)

MetLife is the provider of your Group Term Life Insurance and Accidental Death and Dismemberment (AD&D) Program.

Coverage for Yourself
If you are a newly eligible employee you may elect coverage in $10,000 increments up to a maximum of $300,000. You may choose up to $50,000 guaranteed coverage without completing a Evidence of Insurability (EOI). If you select coverage greater than $50,000 for yourself, you must complete the EOI process to be reviewed by MetLife.Coverage for Your Dependents
You may chose to purchase coverage for your dependents who are eligible for health benefits with the State in $5,000 increments up to half of your coverage amount (up to a maximum of $150,000). You may choose up to $25,000 guaranteed coverage for eligible dependents without an EOI. If you select coverage greater than $25,000 for a dependent, an EOI for that dependent must be completed and reviewed by MetLife.If you have any other questions regarding this program please visit the MetLife SMD website or contact our MetLife dedicated customer service representatives at 1-866-574-2863.

AD&D
The Accidental Death and Dismemberment (AD&D) Plan is available to all Active Employees and their dependents who are eligible for health benefits with the State. The AD&D Plan is offered through MetLife Insurance Company. The plan provides benefits in the event of an accidental death or dismemberment. No medical review is required for enrollment in the plan. This plan will cover you for accidents that occur at work as well as accidents off the job.
You can choose individual or family coverage in an amount equal to
- $100,000
- $200,000
- $300,000
If you choose family coverage, your dependents are covered for a percentage of your benefit amount.
