Immunizations
Required immunizations
Each student must have a completed immunization record on file with the Student Health Center in accordance with the University Immunization Compliance Policy. Documentation of immunization compliance requirements is due by Aug 1 for fall and January 2 for spring.
*The required records MUST BE scanned and uploaded through the Patient Portal. Our system is all electronic. NO mailed, fax or email copies of immunization records are accepted.*
Please allow at least 72 hours during regular business hours after uploading your record for the health center staff to review. You may review the status through the Patient Portal.
Health holds are placed on student records with missing or incomplete documentation. Students will not be able to register for classes until the required health information is complete. Students must login to the Patient Portal to check Medical Clearance status. If status is satisfied, the student is compliant with the health requirements. If the status is not satisfied, the student has an unmet requirement that needs to be completed.
Required immunizations
- Measles-Mumps-Rubella (MMR) vaccination - two doses on or after the first birthday separated by at least one month
- Tetanus-Diphtheria (Td) or Tetanus-Diphtheria-acellular Pertussis (Tdap) within the last ten years
Strongly Recommended immunizations
- COVID-19 vaccine
- Hepatitis B vaccine
- Varicella vaccine
- Gardasil (HPV vaccine)
- Meningococcal vaccine

Required Tuberculosis Screening
Each student must have a completed Tuberculosis Risk Assessment form on file at the Student Health Center. Health holds are placed on student records with missing or incomplete information. Students will not be able to register for classes until the required health information is complete.
Complete the Tuberculosis Risk Assessment form (TB screening) on Patient Portal.
Those who have any positive responses on the tuberculosis risk assessment form must complete the following in the Patient Portal:
- have a PPD (tuberculin) skin test (within six months of attendance and done in the United States) or a blood test for tuberculosis (T-SPOT or QuantiFERON-TB Gold test). An official copy of the lab report must be submitted to the Student Health Center
- Students with a positive PPD skin test (current or a history of) or a positive TB blood test must have a chest x-ray and forward a copy of the official chest x-ray report to the Student Health Center
Vaccines available at the Student Health Center
The following vaccines are available at the Student Health Center by appointment:
- Tdap vaccine (Tetanus-Diphtheria-acellular Pertussis)
- MMR vaccine (Measles-Mumps-Rubella)
- Hepatitis B vaccine
- Influenza vaccine (seasonal)
- PPD (tuberculosis) skin testing is also available at the Student Health Center
