Update on Proof of Vaccination for Fall 2021

By University System of Maryland mandate, Frostburg State University will require all FSU students and employees on campus for the fall 2021 semester to provide proof of being fully vaccinated for COVID-19 or have an approved exemption. Please review the following information:
ALL STUDENTS and EMPLOYEES can provide information starting TODAY from your COVID-19 vaccination card by uploading a photo or file through the COVID-19 Check-In Portal under the “My Vaccinations” menu option. You will need to provide this even if you entered information earlier this spring. The deadline for uploading your proof of vaccination is August 1.
Exemptions
Applying for a religious or medical exemption is handled through the COVID-19 Check-In Portal under the “My Exemptions” menu option. Forms and details can be found in the portal.
Proof-of-Vaccination Cards/Information
If you do not have your vaccination card, you may be able to access your record through your state’s immunization web site. The three most common sites for FSU students and employees are:
Important Dates
- July 1: If you are not currently vaccinated for COVID-19, receiving your first shot or single shot by July 1 or earlier allows you time to schedule your final dose (if two-shot) and upload your information by the August 1 deadline.
- August 1: Students and employees on campus for fall 2021 will need to have your proof of COVID-19 vaccination information or exemption requests entered by August 1.
- August 15: FSU has set August 15 as the date by which you must be fully vaccinated to be on campus for the fall 2021 semester. Note: individuals are considered fully vaccinated 14 days after their second shot for two-dose vaccines (Pfizer or Moderna) or their only shot for the single-dose Johnson & Johnson vaccine.
- August 30: Classes begin for the fall 2021 semester.
