Required Documents


For Academic Acceptance

To continue your application process, the FSU Admissions Office requires the following documents for academic acceptance:

✓ Copy of Passport

✓ All Transcripts

Including high school (only for first year students), and university levels must go through a Course-by-Course foreign credential evaluation report issued by one of the following agencies:

***These reports must be sent directly from the Agency to FSU to be considered Official by the Admissions Office***

✓ Qualifying GPA Scores or ACT/SAT Scores

ONLY if requested by the FSU Admissions Office

✓ LANGUAGE SCORE

Proof of English language proficiency from one of the following testing companies:
(May be waived for students whose primary language is English)

  • DuoLingo - Graduate 100, Undergraduate 85
  • TOEFL - Graduate (550P/213C/79iBT), Undergraduate and Exchange (500P/173C/61iBT)
  • IELTS - Graduate (6.0), Undergraduate and Exchange (5.5)
  • ELS Language Centers - Graduate and Undergraduate Level 112
  • Inlingua English Center - Graduate and Undergraduate Level 8
  • The Language Company (TLC) - Graduate and Undergraduate (Level 9), Exchange (Level 7)
  • LADO International Institute - Graduate and Undergraduate Level 10
  • iTEP - Graduate (4.5), Undergraduate (3.6, no sub-score below 3.5), Exchange (3.5)
  • EIKEN - Graduate (Pre-1), Undergraduate (2A)
  • Evergreen Academy

VISA

The Center for International Education (CIE) is looking forward to assisting you with obtaining your visa! The CIE must collect the following items to issue visa documents:

✓ Official FSU Acceptance letter

(from the Admissions Office)

✓ Financial Certification Form

(download form)

✓ Bank Statement

Bank Statement or Bank Letter showing enough funding for one year of study (Tuition and Fees, Room and Board, Health Insurance, Books, and Personal Expenses)

  • Please request two copies of your bank statement or financial letter. One copy you will take to your Visa appointment, the other will be given to the CIE Office upon your arrival at FSU.
  • The name on the bank statement must match the name of the person signing the Financial Certification Form.
  • The bank statement must include the bank’s contact information (branch location, address and phone number).
  • The bank statement must be less than six months old at the time FSU issues the I-20 document.
  • Bank letters must be signed by a bank officer and printed on official letterhead paper.
  • Photocopies and account statements printed off the internet are not acceptable.

For Transfer Students

✓ Transcripts

From all previous universities, colleges, and IEPs attended in the U.S.

✓ Transfer Eligibility Form

(Completed and signed by the P/DSO at previous institution attended)

  • VISA - most recent visa used to enter the U.S.
  • I-94 Report - (print out from https://i94.cbp.dhs.gov/home)
  • I-20 DOCUMENTS - All previous I-20 documents issued for study in the U.S.
  • NOTICES OF ACTION - all previous Notices of Action (I-797A) issued by USCIS for an Application to Extend/ Change Non-Immigrant Status (if applicable)
  • EAD CARDS - All previous Employment Authorization Documents (EAD cards) for employment authorization for Optional Practical Training (OPT) or Severe Economic Hardship (if applicable)
